I the undersigned agree to the following:

1.That I will receive one hour of musical play class, followed by one hour’s use of the party room, for the purposes of eating, present opening, etc. (Additional hours may be purchased at a rate of $50/ per hour or $25 per 1/2 hour.)

2.That I will notify the teacher if there is any reason why any of the guests should not participate fully in the party. (injury, illness, disability, etc.)

3.That children under the age of 3 will be accompanied by a care giver.

4.That at no time shall I put tape or pins on the walls at the Jump Start Studio. (Use of large bulletin board ok.)

5That I will pay a non-refundable deposit of $50 by credit card, at the time of registration to reserve my party. (The balance may be paid by cash, cheque or credit card at the start of your party.) This applies to the total cost of the party. In the event of illness, another party date will be arranged, if at least 24 hours notice of the illness has been given.

6.That I will pay the remaining balance owing on the day of the party, before the start of the party, (cash, cheque or credit card) . I understand I will be billed at a rate of $15 per 15 minutes for any extra time outside of the 2 hours which is incurred, or double that rate if my guest's late departure delays the party following mine.

7.That I am responsible for any damages to the studio incurred by myself or one of my guests.

8.That Jump Start Music & Movement cannot be held liable for any loss or injuries related directly or indirectly to participation in any Jump Start program.

**Please note that the studio will be open approximately 15 minutes before the party, in order for you to set up. You may wish to encourage your guests to come five or ten minutes early to take off coats and prepare for the fun.

**At the close of your booked time, please move your guests to the waiting area, so preparation may be made for the next party. Your guests should be finished in the waiting area within 15 minutes of your party to make room for the next party's guests. After that, additional charges are incurred.

***Please respect the time you have booked, as parties often follow yours. Additional time which has not been booked will be billed at a rate of $15 for each 15 minutes. If the party after yours is delayed on your account, please note that you will be billed at double the extra time rate ($30 for each 15 minutes.) If you feel your party cannot be complete in 2 hours, it is more economical to book the extra time ahead at the regular rate.
